When you need this form

Exit corridors get blocked. Stock lands in a fire stair, a pallet sits against an exit door, a self-closer stops latching, an exit sign goes dark. Paths-of-travel checks catch that before it becomes an evacuation problem. Most states require building owners to certify their essential safety measures periodically, and this check is the evidence layer underneath that certification, gathered by whoever walks the building rather than the certifier.

Certifiers and building owners want more than a tick sheet at renewal time. A photo of the obstruction, the timestamp it was found, the note that it was cleared within the hour and the follow-up signature tell a far stronger story. It also protects the contractor: you can show the blockage was reported, and who was told.

What good looks like

• Every exit route walked end to end, including external discharge points.
• Obstructions photographed in place before they are moved.
• Exit doors tested: latching, hardware, single action, opening in the right direction.
• Exit and emergency lighting condition noted per fitting, not per floor.
• Corrective actions assigned with a name, due date and closing photo.
