When you need this form

Fire doors get wedged open, damaged by trolleys and re-hung with the wrong hardware. Penetrations get cut for new cabling and never resealed. Both undo the compartmentation the building depends on, and both are found by walking the site door by door and riser by riser. Inspections run on the routine cycle, after fit-outs and cabling works, and when evidence is being gathered for a statement or readiness review.

Door tags and unsealed penetrations only mean something with a photo and a location. A live inspection numbers each door, records the defect against it, and carries the image of the missing seal or the damaged tag. Next inspection, the same door is already there with its history, so repeat failures are obvious and the contractor who left the penetration open is easy to identify.

What good looks like

• Each door identified by number and location, not counted in bulk.
• Self-closing operation and latching tested, wedges and props recorded.
• Seals, gaps, damage and missing tags photographed against the door.
• Service penetrations checked for correct sealing after cabling or plumbing works.
• Defects rated, corrective actions raised and compliance status stated.
