When you need this form

Risk assessment is the backbone of any facilities safety system. Use this when you take on a new building, before a client safety audit, when a task or plant item changes, and on a scheduled review cycle. It records the hazard, its condition, a risk rating and the control that follows. Most states require the assessment to exist and to be reviewed, not just written once.

Spreadsheets go stale the day they are made. A live assessment carries the date, the assessor, a photo of the actual hazard and the action assigned against it. Auditors look for that closing loop: risk identified, control applied, rating reviewed. A dated trail of reassessments proves the system runs, which is what a client audit is actually testing.

What good looks like

• Each hazard described by location and task, not a generic category.
• Risk rated before and after controls, with the residual rating recorded.
• Controls named in order: eliminate, substitute, engineer, administer, then PPE.
• Photo of the actual hazard attached, not a stock illustration.
• Action owner and due date set, with a review date for reassessment.
