When you need this form

Building management systems throw alarms constantly, and most mean nothing on their own. The log is where a technician separates noise from a real plant fault: chiller trips, pump failures, temperature drift, ventilation faults and cooling tower alarms. Recording triage and response protects the building's occupants and the equipment, and gives the owner the fault history that drives maintenance planning and warranty claims.

Owners judge mechanical contractors on response, not intent. A live log timestamps when the alarm came in, who triaged it, what was done and when the plant returned to normal, then hands over cleanly at shift change. Screenshots and photos attach to the entry. Six months later that history is what supports a warranty claim or a capital replacement recommendation.

• Triage outcome: nuisance alarm, monitor, or genuine fault requiring attendance.
