When you need this form

Most cleaning contracts are won on scope and kept on evidence. This checklist covers the routine service run: tasks completed, quality checks, hazards found and anything left undone. It carries across aged care, childcare, airports, pools, bank branches and call centres, where infection control, restricted access or chemical risk raise the bar. Auditors and facility managers expect the same record every visit.

Word documents get backdated. Live records cannot. Each completed checklist carries the time it was filled, the site it was filled at, the cleaner who signed it and photos of the actual result. That turns a monthly service report into an audit trail, which is what accreditation assessors, infection-control reviewers and tender panels ask to see.

What good looks like

• Every scheduled task marked complete, skipped or blocked, with a reason.
• Quality check ratings against the site scope, not a generic pass.
• Photo evidence for each area cleaned, taken after the work.
• Hazards and exceptions logged with the corrective action already assigned.
• Cleaner and site contact sign-off, timestamped and GPS-pinned.
