When you need this form

Planned maintenance covers everything that fails quietly: switchboards, air handling units, pumps, playgrounds, trees, lighting, doors and plant rooms. Work runs to a schedule, monthly, quarterly or annually, and the checklist is the schedule made real. Most client contracts and insurance arrangements assume this rhythm exists, and outdoor and public assets carry the sharpest liability, because a defect nobody recorded is a defect nobody fixed.

The value sits in the exceptions. A live checklist turns a failed check into a defect with a photo, a location and a follow-up action, then carries it into the next visit instead of losing it on a clipboard. Clients reviewing a monthly report can see which assets were attended, which are trending down, and which need capital. Paper cannot do any of that.

What good looks like

• Asset identified by tag and location, not a vague area name.
• Every scheduled check marked pass, fail or not applicable with a reason.
• Condition rating recorded per asset, so trends show across visits.
• Defects photographed and raised as follow-up work, with priority set.
• Technician name, attendance time and site sign-off closing the visit.
