When you need this form

No contractor should touch a site before they have been walked through it. Site rules, emergency exits, isolation points, permit requirements, who to call. An induction sign-off is the record that the walk-through happened and that the person understood it. Asset owners pushing compliance onto their contractor base treat it as the gate: no induction, no access, no exceptions.

The weakness of a paper induction register is that it proves a signature, not a person or a time. A live sign-off stamps who signed, when they arrived, where they were standing and which version of the site rules they saw. It also carries their photo and licence, so the person on site is provably the person you inducted.

What good looks like

  • Site, date and the inducting supervisor named on every record.
  • Each induction topic acknowledged individually, not one blanket tick.
  • Visitor or contractor identity captured with photo and company details.
  • Licences, tickets and permits sighted and attached as evidence.
  • Time on and off site recorded, with a signature at sign-on.

Induction sign-off, answered

Is this a PDF or Word template?

Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.

When does a contractor need a site induction?

Before first entry to the site, and again after any material change to site rules, hazards or the scope of work. Many asset owners also require a fresh induction each year.

What should a site induction cover?

Site layout and access, known hazards, emergency procedures and exits, permit and isolation requirements, PPE, reporting lines and who to contact. Each point should be acknowledged and signed by the worker.
